I'm managing some mail server. One of our costumers called because he was recieving "a couple of spam emails" and asked if there was something wrong on the server because he didn't anything.
There was 700k mails in the outgoing queue of his account because the source of spam was him. What he saw was the "undelivered mail" and the server notifying him that his account was blocked.

In the end, he configured the mail on a computer full of viruses that sniffed the password and communicated on a botnet.
