When team members want to discuss an issue for so long that if you spent a quarter of the time working, you would have completed the task.

When your pm forces you to push new features out too quickly and you're slowly digging yourself into a messy hell, knowing you have to one day refactor thousands of lines of code.

When a client starts saying that should be easy to do when talking about coding a new feature.

Estimations:

2 days = One day of actual coding, one day of doing something completely unrelated.
3 days = One week
5 days = Two weeks
7 days = Forget about it...
